Golf Tournament Information

• Organize your own five-person team. (men, women, or mixed)
• Field Limit is 18 teams.
• To register, complete the team application (and handicaps) below.
• For singles or incomplete teams, please contact Yuliy Fedoryshyn at yuliy@bghcfoundation.com.
  The committee will try to connect you with other single players or incomplete teams if there are any available.
• $500 per golfer. For payment, turn in or mail your check (made payable to Boca Grande Health Clinic Foundation or BGHCF) to:
